Managers are required to collect and keep medical forms with the team at all times. The Code of Conducts and FOIP are to be signed and dated by all affected parties (ie. Social Media - player and/or parent, Player - player and/or parent, Parent - all parents), collected and handed in to the SMRA Secretary. Volunteer bond is a $250 cheque post dated for April 30 of the year of play for the season, also handed in to the SMRA Secretary. Please collect all of these items, place them in an envelope and make arrangements to deliver to the SMRA Secretary.
